In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Wilmar,MN

In-Home Health clinician role providing in-home health evaluations for Nurse Practitioners or Physician Assistants. Conduct 45-minute visits to members’ homes, including medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by health plan. Use an iPad and established clinical workflow; not required to prescribe medications, order labs, or alter current treatments. Responsibilities include building trusted relationships, traveling locally within licensed states, conducting virtual visits as needed, and delivering Diagnostic & Preventive Services. Primary workplace is the member’s home with exposure to varied environments; travel to Wilmar, MN and surrounding areas. Part-time role with benefits and malpractice coverage; anticipated weekly hours ~40. Requires active, unrestricted licenses, board certification, and multi-state licenses preferred.
