Job Summary
In-Home Health Evaluations by a Nurse Practitioner or Physician Assistant to provide personalized evaluations in members' homes. The role involves 45-minute visits with health questions, medication and medical history reviews, physical evaluations, and optional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening) ordered by health plans. You will use an iPad with a clinical workflow, conduct virtual visits as needed, and travel locally to members' homes. Primary workplace is the member's residence; environments may include pets or secondhand smoke. Requires active, unrestricted licensure and board certification; multi-state licenses preferred. The role supports building trusted relationships to improve health outcomes and requires flexibility to travel within licensed states. Part-time employment with select benefits and malpractice coverage is offered.
