In-Home Health Evaluations for Nurse Practitioner or Physician Assistant (Part Time) with Signify Health, traveling to member homes to perform 45-minute visits including medication and medical history review, physical evaluation, and optional diagnostic tests. Use an iPad and a clinical workflow; no prescribing or altering treatment during visits. Role focuses on connecting members with appropriate care through in-home assessments and bridging gaps in care, with flexibility to travel locally within licensed states and to conduct virtual visits as needed.
