In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Wilmar,MN

Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations in members' residences, traveling locally to Wilmar, MN and surrounding areas and Charleston, WV as needed. Responsibilities include 45-minute visits with health questions, medication and medical history review, physical evaluation, and possible diagnostic tests per health plan orders; use of an iPad and a streamlined clinical workflow; no prescribing or altering treatment during visits; occasional virtual visits; and delivering diagnostic and preventive services (e.g., spirometry, DEE, bone density screening). Requires active licenses, board certification, and ability to cover primary locations; offers selective benefits for part-time employees; anticipated weekly hours ~40.
