In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Wilmar,MN

Part-time clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for Signify Health as part of CVS Health, traveling locally to patients’ homes in Wilmar, MN. Each 45-minute visit includes medication and medical history review, physical evaluation, and, if ordered by the patient’s health plan, additional diagnostic tests (e.g., spirometry, bone density screening). Use an iPad and clinical workflow; not required to prescribe medicines or alter current treatment. Must hold an active, unrestricted license in the coverage area and board certification; multi-state licenses are preferred. Primary workplace is the patient’s home; flexibility to travel to surrounding locations and conduct virtual visits as needed.
