Job Summary
Provide in-home health evaluations for health-plan members, delivering flexible, personalized assessments at patients' residences. Responsibilities include a medication and medical history review, physical evaluation, and, if ordered by the plan, diagnostic tests (e.g., spirometry, bone density screening). Use an iPad and a clinical workflow to guide each evaluation; clinicians are not required to prescribe medications, order labs, or alter current treatment plans. Travel locally to member homes, conduct virtual visits as needed, and join a national network of clinicians to improve health outcomes. Requires NP or PA with active license and board certification; multi-state licenses are preferred; part-time schedule with anticipated 40 hours weekly in Wilmar, MN and surrounding locations.
