Job Summary
Part-time In-Home Health Clinician role delivering in-home health evaluations for Signify Health (Nurse Practitioner or Physician Assistant). Conduct ~45-minute visits, review medications/history, perform physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., spirometry, bone density screening). Use an iPad and a clinical workflow; not required to prescribe medications or order labs. Travel locally to patients’ homes; flexibility to conduct virtual visits as needed. Primary workplace is members’ residences; environment may include pets or other household conditions. Board-certified NPs or PAs with active licenses in the coverage area; multi-state licenses preferred. Anticipated weekly hours: 40.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover Wilmar and surrounding locations
- Nurse Practitioner or Physician Assistant credential
- Experience in in-home health evaluations or clinical home visits (preferred)
- Ability to travel locally as needed
- Comfort with using an iPad and clinical workflow
- Knowledge of spirometry, DEE, bone density screening (preferred)
