Job Summary
In-Home Health Evaluations for Nurse Practitioner or Physician Assistant (Part Time) serving Wilmar, MN and surrounding locations. Conduct 45-minute in-home visits using an iPad and clinical workflow to review medication and medical history, perform physical evaluations, and, when ordered by the health plan, complete optional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Roles involve flexible local travel and virtual visits as needed; primary workplace is members' homes with diverse environments. Requires active, unrestricted NP or PA license with board certification and, ideally, multi-state licenses. Responsibilities emphasize building trusted relationships to improve health outcomes, providing connections to appropriate care, and delivering Diagnostic & Preventive Services as indicated. Minimum anticipated weekly hours: 40; pay range provided is hourly with base compensation determined by geography and experience.
