Job Summary
In-Home Health Clinician role for Nurse Practitioner or Physician Assistant (Part Time) with Signify Health/ CVS Health. Conduct 45-minute in-home health evaluations in patients' residences, including medication/medical history review, physical evaluation, and potential diagnostic tests as ordered by the health plan. Travel locally to Wilmar, MN and surrounding areas (and Albuquerque, NM as indicated by postings), using an iPad-based clinical workflow; no initiation of prescriptions or changes to treatment during visits. Requires active, unrestricted licensure, board certification, and ability to cover primary locations with occasional virtual visits; part-time schedule with anticipated 40 hours weekly.
