In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Wilmar,MN

Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in members' homes. Visits last about 45 minutes and include medication/medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and a clinical workflow; not required to prescribe medicine, order labs, or alter current treatment. Responsibilities include engaging with patients at home, building trusted relationships, and delivering flexible, comprehensive health evaluations directly to patients. Travel locally within licensed states as needed; virtual visits available; primary workplace is the member's home. Qualified candidates hold an active, unrestricted license(s), board certification, and preferably multi-state licenses. Primary locations include Wilmar, MN and surrounding areas.
