Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. Conduct 45-minute visits in members' homes, including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams, bone density screening) as ordered by health plans. Use an iPad-based clinical workflow; not required to prescribe meds, order labs, or alter treatments. Travel locally to member homes; may conduct virtual visits as needed. Primary workplace is the member's home; environments may contain pets or smoke. Eligible clinicians are active NP or PA licenses with board certification; role supports flexible scheduling and comprehensive benefits.
