Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for Signify Health. Visit patients in their homes for about 45 minutes, including medication and medical history review, physical evaluation, and optional diagnostic tests. Use an iPad-based clinical workflow; not required to prescribe meds, order lab tests, or alter current treatment. Travel locally to patient homes; conduct virtual visits as needed; provide Diagnostic & Preventive Services as indicated. Primary workplace is the home of members; environments may include pets, secondhand smoke, and other irritants. Active, unrestricted license, board certification, and ability to cover Wilmar, MN area are required; multi-state licenses preferred. Anticipated 40 hours per week with part-time status.
