In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Wilmar,MN

In-Home Health Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations within the Wilmar, MN area. Responsibilities include conducting 45-minute visits with patients at their homes, performing medication and medical history reviews, physical evaluations, and, if ordered by the patient’s health plan, additional diagnostic tests (e.g., spirometry, aging-related screenings). Use an iPad and a clinical workflow to guide evaluations; no prescribing, ordering labs, or altering current treatment plans is required. Travel locally to member homes; virtual visits may be conducted as needed. Provide Diagnostic & Preventive Services and support care coordination to bridge gaps in care. Part-time role with anticipated weekly hours of 40 and a pay range of $41.85–$90.13 hourly. Eligible for select benefits; malpractce insurance coverage during Signify Health activities; multi-state licenses preferred. Primary locations include Wilmar and surrounding areas; must uphold active, unrestricted licensure and board certification.
