In-Home Health Clinician role for Nurse Practitioner or Physician Assistant (Part Time) delivering in-home health evaluations to members in their residences. Responsibilities include conducting 45-minute visits with medication and medical history review, physical evaluation, and optional diagnostic tests per health plan orders; using an iPad and clinical workflow without prescribing medications or altering current treatment regimens. Travel locally within licensed states; conduct virtual visits as needed; provide Diagnostic & Preventive Services such as spirometry, bone density screening, and eye exams; primary workplace is member’s home; environment may include pets and allergens. Requires active NP/PA licensure, board certification, and ability to cover Wilmar, MN and surrounding areas; multi-state licenses preferred; part-time schedule with anticipated weekly hours around 40 at pay range $41.85–$90.13 per hour.
