Job Summary
Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow for each visit while connecting one-on-one with patients to answer health questions and guide them toward better self-care. Fulfill all Signify Health services as indicated, including preventive screenings, within a flexible schedule that may require local travel and virtual visits. Perform duties safely in diverse home environments that may contain pets, smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Must be flexible to travel locally and within licensed states, as needed
