Job Summary
In-Home Health Clinician role for Nurse Practitioner or Physician Assistant (Part Time) in Colorado. You will conduct in-home health evaluations for members in their homes, perform medication and medical history reviews, physical evaluations, and order/perform additional diagnostic tests as indicated by health plans. The visit lasts about 45 minutes and utilizes an iPad with an intuitive clinical workflow. Travel locally within licensed states and virtual visits as needed. Role emphasizes building trusted relationships, bridging gaps in care, and delivering flexible, comprehensive, personalized health evaluations directly at patients' doors. Requires active NP or PA license with board certification and the ability to cover primary Colorado locations; provides malpractice coverage and benefits for part-time staff.
