In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Pueblo,CO

Provide in-home health evaluations as a Part-Time Clinician (Nurse Practitioner or Physician Assistant) across prescribed Colorado locations, delivering one-on-one visits in patients' homes. Conduct medication and medical history review, physical evaluation, and, when ordered by the patient’s health plan, additional diagnostic tests. Utilize an iPad-based clinical workflow during evaluations and avoid prescribing, ordering labs, or altering current treatment regimens. Travel locally to designated primary locations, perform virtual visits as needed, and deliver Diagnostic & Preventive Services (e.g., spirometry, bone density screening) as indicated. Must hold active licenses, board certification, and, ideally, multi-state licensure.
