Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in patients' homes, engaging with people to bridge gaps in care. Visit lasts ~45 minutes, includes medication and medical history review, physical evaluation, and additional optional diagnostic tests. Use an iPad and clinical workflow; not required to prescribe meds, order labs, or alter current treatment. Travel locally within licensed states; virtual visits as needed. Provide Diagnostic & Preventive Services such as spirometry and bone density screening.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
