Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations in Plattsburgh, NY. Visits last about 45 minutes and include medication/history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry). Clinician will use an iPad and follow a clinical workflow; prescriptive changes or lab orders are not required.
