Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to connect one-on-one with patients, review their living conditions, and provide education on health management without prescribing medication or altering treatment regimens. Perform virtual visits as needed while traveling locally within licensed states to serve members in Plattsburgh and the surrounding area. Maintain flexibility to cover diverse home environments that may include pets or allergens. Work part-time on a 29.5-hour weekly schedule with base pay ranging from $46.03 to $99.14 hourly.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Plattsburgh and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
