Job Summary
Part-time In-Home Health Clinician (Nurse Practitioner or Physician Assistant) delivering home-based evaluations. Conduct 45-minute visits including health questions, medication/medical history review, physical evaluation, and optional tests (e.g., spirometry, bone density screening) as directed by members’ health plans. Utilize an iPad and a streamlined clinical workflow; no prescribing, labs, or treatment changes needed during visits. Responsibilities include building trust with members, visiting across multiple locations in NY (Oneida/Syracuse area) with travel to homes, and facilitating connections to appropriate care. Requires active NP/PA licensure, board certification, and ability to work in members’ homes, with flexibility for local travel and occasional virtual visits. Preferred multi-state licenses and ongoing participation in a national clinician network.
Required Qualifications
- Active, unrestricted license in the scope of practice for NP or PA (NY)
- Board certification (NP or PA)
- Ability to cover Oneida/Syracuse, NY locations
- Multi-state licenses (preferred)
- Proficiency with diagnostic and preventive services where applicable (spirometry, bone density screening, etc.)
- Experience in in-home health evaluations or home-visit clinical settings (preferred)
- Ability to travel locally within licensed states and conduct virtual visits as needed
