Job Summary
Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and diagnostic tests such as spirometry or diabetic eye exams. Perform virtual visits as needed and utilize an iPad with an intuitive clinical workflow for each assessment. Work with Signify Health as a clinician colleague to bridge care gaps and provide personalized health education directly in member homes. Be flexible to travel locally within licensed states and accommodate diverse home environments that may contain pets or allergens. This part-time role in Okanogan, WA, requires an active, unrestricted license and board certification.

- Board certification (required)
- Able to cover the following primary locations: Okanogan and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
