Conduct 45-minute visits including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density, DEE) as ordered by the member’s health plan. Role involves traveling locally, visiting members in their homes, using an iPad and clinical workflow, providing education, and delivering comprehensive, personalized health evaluations without changing current treatment regimens. Requires NP or PA credentials with active license(s) and board certification; multi-state licenses preferred. Primary workplace is the member’s home; focus on building trusted relationships to bridge gaps in care across a national clinician network.
