Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to perform these visits while connecting one-on-one with patients in their private residences. Fulfill specified Signify Health services including diagnostic and preventive care products, and conduct virtual visits as needed. Travel locally and within licensed states as required to meet patient needs. This part-time role involves 29.5 weekly hours with a base hourly rate ranging from $41.85 to $90.13. Candidates must hold an active, unrestricted license in the coverage area and complete 2080 hours of post-graduate collaborative practice.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Filing of the appropriate attestation with the state of Minnesota
- Board certification
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
