Job Summary
Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical assessments, and ordered diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to connect one-on-one with patients, answer health questions, and provide education on their unique needs. Travel locally within licensed states and perform virtual visits as required. Work part-time with a schedule of approximately 29.5 weekly hours. Candidates must be comfortable performing duties in diverse home environments that may contain pets, smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Filing of the appropriate attestation with the state of Minnesota
- Board certification
- Multi-state licenses
