Job Summary
In-Home Health Evaluations for Nurse Practitioner or Physician Assistant in a part-time role. Conduct 45-minute visits in members' homes using an iPad and a clinical workflow to review medication/history, perform physical evaluation, and provide indicated diagnostic services (e.g., spirometry, bone density screening) when ordered by the member’s health plan. Responsibilities include building trusted relationships with health plan members, visiting millions nationwide, traveling locally within licensed states as needed, and conducting virtual visits as required. The clinician will not prescribe medications or alter current treatment during visits but supports diagnostic and preventive services. Required qualifications include an active, unrestricted license in the coverage area, completion of 2080 post-graduate collaborative practice hours, board certification, and multi-state licenses (preferred). The role emphasizes home-based care in diverse environments and offers select benefits for part-time employees. The anticipated weekly hours are 40.
