In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) Marshall,MN

Part-time clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for Signify Health, visiting patients in their homes for about 45 minutes per visit. Responsibilities include medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Use an iPad and a clinical workflow; not required to prescribe medications, order labs, or alter current treatment. Travel locally as needed to Marshall, MN, and surrounding area; conduct virtual visits as needed; primary workplace is the patient’s home. Responsibilities also include engaging with health plan members, building trusted relationships, and delivering flexible, comprehensive, personalized health evaluations to improve health outcomes.
