In-Home Health Evaluations for Nurse Practitioner or Physician Assistant (Part Time) delivering 45-minute home visits to connect members with personalized care, performing medication/history reviews, physical exams, and, when ordered, optional diagnostic tests (e.g., spirometry). Travel locally within licensed states, with virtual visits as needed, using an iPad and streamlined clinical workflow; primary workplace is the member's home. Requires active NP/PA license(s), board certification, and post-graduate clinical hours; compensation includes a part-time hourly range and benefits.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Board certification (required)
- Multi-state licenses (preferred)
