Job Summary
Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to deliver flexible, personalized care while engaging one-on-one with patients in their private residences. Perform virtual visits as needed and travel locally within licensed states to visit millions of people nationwide. Maintain active, unrestricted licenses in the coverage area and complete 2080 hours of post-graduate collaborative practice hours. Work part-time, approximately 29.5 hours weekly, with a closing date of December 31, 2026.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Filing of the appropriate attestation with the state of Minnesota
- Board certification
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
