Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for health plan members in their homes. Visit lasts about 45 minutes and includes medication/history review, physical evaluation, and possible diagnostic tests ordered by the health plan. Use an iPad and clinical workflow; not required to prescribe medications or alter current treatment. Role supports visiting people nationwide through Signify Health within CVS Health; anticipated weekly hours ~29.5. Employment includes select benefits and malpractice insurance coverage; environment involves home-based visits with potential exposure to household pets and allergens. Responsibilities include building trusted relationships, connecting members to appropriate care, and delivering diagnostic/preventive services as indicated (e.g., spirometry).
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Board certification (required)
- Multi-state licenses (preferred)
