In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Colorado Springs, CO

Part-time clinician (Nurse Practitioner or Physician Assistant) delivering in-home health evaluations for Signify Health/CVS Health. The role involves visiting health plan members in their homes, conducting medication and medical history reviews, performing physical evaluations, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry). You will use an iPad and follow a clinical workflow; prescribing medications, ordering labs, or altering current treatment is not required. Primary duties include building trusted relationships, visiting members nationwide as needed, and providing Diagnostic & Preventive Services as indicated. Primary work location is the private residence of health plan members with travel to local areas; virtual visits are possible. Qualified applicants are NP or PA with active, unrestricted licenses, board certification, and the ability to cover Colorado Springs, CO area; part-time schedule approximately 29.5 hours per week, with a pay range of $41.85-$90.13 per hour.
