Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical examinations, and diagnostic tests such as spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to deliver preventive and diagnostic services while connecting with patients in their private residences. Perform virtual visits as needed and travel locally within licensed states to serve members in the Bronx and surrounding areas. Maintain board certification and an active unrestricted license to provide these direct care services.

- Able to cover the following primary locations: Bronx, New York and surrounding area
- Be flexible to travel locally and within licensed states, as needed

- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
