Job Summary
In-Home Health Clinician (Nurse Practitioner or Physician Assistant) in Washington, responsible for conducting in-home health evaluations for Signify Health by CVS Health. Roles include 45-minute visits with medication and medical history review, physical evaluation, optional diagnostic tests, use of an iPad, and adherence to a clinical workflow without prescribing or altering current treatment. Flexible to travel locally within Washington and conduct virtual visits as needed; primary workplace is members’ homes. Full-time position with benefits including health/dental/vision, PTO, CEU allowance, 401K, malpractice coverage, and additional perks. Must hold active, unrestricted licenses, board certification, and preferably multi-state licenses. Anticipated weekly hours: 40; pay range: $95,738–$206,206, with compensation influenced by experience, geography, and other factors.
