Job Summary
Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations through Signify Health in Washington. Conduct ~45-minute in-home visits including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by the health plan. Use an iPad and clinical workflow; not required to prescribe or alter treatment. Travel locally within licensed states, with virtual visits as needed. Primary workplace is members' homes; environment may include pets or allergens. Requires active, unrestricted NP or PA license and board certification; multi-state license preferred. Anticipated weekly hours: 40. Pay range for base salary: $95,738–$206,206; comprehensive benefits provided. Positions available across all counties within the state of Washington; consider Portland, OR location as indicated by postings. Candidates must be able to perform duties safely in private homes.
