In-Home Health - Nurse Practitioner or Physician Assistant (Full Time)-Washington (Traveler)

Full-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for Signify Health under CVS Health. Visits about 45 minutes, including medication and medical history review, physical evaluation, and, when ordered by the member’s health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). You’ll use an iPad and a clinical workflow; prescribing, ordering labs, or altering current treatment is not required. Responsibilities include building trusted relationships to improve health outcomes and visiting members nationwide with flexibility to travel locally and within licensed states; conduct virtual visits as needed. Primary workplace is the member’s home. Benefits include health/dental/vision, PTO, CEU allowance, 401K, malpractice coverage. Required qualifications: active, unrestricted licenses in the coverage area, board certification; multi-state licenses preferred. Primary locations: ALL COUNTIES WITHIN THE STATE OF WASHINGTON. Anticipated weekly hours: 40. Pay range: 95,738.00 - 206,206.00. This role offers additional reimbursement opportunities for DPS and other criteria.
