In-Home Health Clinician role providing in-home health evaluations for patients, servicing Washington state counties and travel-ready across licensed states. Nurse Practitioner or Physician Assistant needed for full-time roles to conduct 45-minute home visits, perform medical history reviews, physical evaluations, and coordinate diagnostics as ordered by health plans. Clinicians will use an iPad with an intuitive workflow, will not prescribe or alter treatment during visits, and will conduct virtual visits as needed. Requires active NP/PA license, board certification, and ability to travel locally; primary work environments are member homes with varied conditions.
