Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, engaging patients in their residences to review medication history, perform physical exams, and administer diagnostic tests such as spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to deliver preventive and diagnostic services, including bone density screening, while connecting patients to appropriate care plans. Perform virtual visits as needed and maintain flexibility to travel locally within licensed states. Work full-time hours in diverse home environments, requiring comfort with household pets, secondhand smoke, and airborne allergens. This role supports Signify Health's mission to simplify healthcare by providing personalized, compassionate care directly to individuals and families across the St. Cloud area.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to cover the following primary locations: St. Cloud and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
