Job Summary
In-home health clinician role for Nurse Practitioner or Physician Assistant delivering 45-minute in-home health evaluations to health plan members in Plattsburgh, NY. Responsibilities include medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry). Clinician uses an iPad with a defined clinical workflow and is not required to prescribe medications or alter current treatments. Must be flexible to travel locally and within licensed states, conduct virtual visits as needed, and provide Diagnostic & Preventive Services. Primary workplace is the member’s home; visits may occur across the national Signify Health network, with full-time benefits and a competitive pay range. Requirements include active, unrestricted licenses in the coverage area, board certification, and willingness to cover Plattsburgh and surrounding area; multi-state licenses are preferred. Anticipated 40-hour work week; full-time position.
