Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical examinations, and additional diagnostic tests such as spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to perform these visits while connecting one-on-one with patients to answer health questions and guide them toward better self-care. Fulfill all Signify Health services including diagnostic and preventive products, conduct virtual visits as needed, and travel locally within licensed states. Work full-time hours in the private residences of health plan members, maintaining flexibility to cover New London and surrounding areas.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
