Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to deliver comprehensive preventive and diagnostic services while engaging patients one-on-one in their private residences. Perform virtual visits as needed and travel locally within licensed states to serve the New London area. Maintain flexibility to adapt to diverse home environments, including those with pets or allergens, while providing education and bridging care gaps without prescribing or altering existing treatment regimens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: New London and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
