Job Summary
In-Home Health Evaluations by a Nurse Practitioner or Physician Assistant, delivering patient-centered health assessments in members' homes. The role involves 40 hours per week, travel within the licensed states, conducting physical evaluations, medication and medical history reviews, and, when indicated by the health plan, additional diagnostic tests (e.g., spirometry, eye exams, bone density screening). Virtual visits are available as needed. Primary workplace is the member's residence; roles include building trusted relationships, connecting patients to appropriate care, and delivering comprehensive, personalized health evaluations. Requires active, unrestricted licensure in the coverage area, board certification, and, ideally, multi-state licensure. Location options include New London, CT and surrounding area, and White Plains, NY; local travel required. Benefits and compensation are provided with full-time status, anticipated weekly hours of 40, and a salary range of $95,738.00 to $206,206.00, plus benefits.
