Job Summary
In-Home Health Evaluations for health plan members by a Nurse Practitioner or Physician Assistant. The clinician conducts medication and medical history reviews, physical evaluations, and may perform additional diagnostic tests (e.g., spirometry) as ordered by the health plan. Visits occur in the member’s home, typically about 45 minutes, using an iPad-based clinical workflow; no obligation to prescribe medications, order labs, or alter current treatments. Requires board certification and active licenses in the coverage area, with the ability to travel locally across assigned CT regions.
