Job Summary
Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical assessments, and ordered diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow for each visit while connecting one-on-one with patients to answer health questions and guide them toward better self-care. Perform virtual visits as needed and travel locally within licensed states to reach health plan members in their private residences. Fulfill all Signify Health services including diagnostic and preventive care products.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Filing of the appropriate attestation with the state of Minnesota
- Board certification
- Multi-state licenses
