Job Summary
In-home health evaluations performed by a Nurse Practitioner or Physician Assistant, with a focus on comprehensive, personalized health assessments delivered in patients' homes. Responsibilities include patient interviews, health history reviews, physical evaluations, optional diagnostic testing (e.g., spirometry), and coordination with health plans to connect members with appropriate care. Requires active NP/PA licensure and board certification; multistate licenses preferred. Full-time role with travel within licensed states and occasional virtual visits, utilizing an iPad and standardized clinical workflow. Located in or servicing New York City area (NYC, NY) with statewide travel as needed.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multistate licenses (preferred)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA
- Ability to travel locally within licensed states
- 40 hours per week expected
- In-home clinical care experience preferred
