Job Summary
In-Home Health Clinician role for Nurse Practitioner or Physician Assistant delivering in-home health evaluations to members in Minneapolis, MN and surrounding areas. Responsibilities include conducting 45-minute visits, reviewing medication and medical history, performing physical evaluations, and providing diagnostic and preventive services as indicated (e.g., spirometry, bone density screening) using an iPad-based clinical workflow. Travel locally within licensed states and virtual visits as needed. The role emphasizes building trusted relationships, improving health outcomes, and delivering comprehensive, personalized health evaluations in members' homes. Requires active NP/PA license with board certification, post-graduate practice hours, and (preferred) multi-state licensure; typical weekly hours are 40 with full benefits including health, dental, vision, PTO, 401K, CEU allowance, and malpractice coverage.
Required Qualifications

- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
