In-Home Health NP or PA Part Time providing in-home health evaluations to patients in their own residences, including history, physicals, and patient education with a focus on bridging gaps in care using a structured clinical workflow. Responsibilities include conducting 45-minute visits, performing medication and medical history reviews, physical evaluations, optional diagnostics per health plan (e.g., spirometry, eye exams), and utilizing an iPad for documentation. Opportunities include virtual visits when needed, visiting patients locally, and delivering Diagnostic & Preventive Services as indicated. Requires active medical license, board certification, and ability to travel locally; multi-state licenses preferred.
