Job Summary
Provide in-home health evaluations for health plan members in Bridgeport and surrounding area. Conduct 45-minute visits including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening) as ordered by the health plan. Use an iPad and a clinical workflow; not required to prescribe medicine, order labs, or alter existing treatment plans. Travel locally to member homes; virtual visits as needed. Eligible full-time clinician will be a Nurse Practitioner or Physician Assistant with active, unrestricted licensure and board certification.
