Job Summary
In-Home Health Clinician (Nurse Practitioner or Physician Assistant) provided by Signify Health/ CVS Health. This Full-time role delivers in-home health evaluations to members in their residences, including medication/medical history review, physical evaluation, and optional diagnostic tests as ordered by the health plan. Responsibilities include building trusted relationships with patients, conducting 45-minute visits, utilizing iPad-based workflows, and performing diagnostics and preventive services (e.g., spirometry, bone density screening) without altering current treatments. Travel locally within the Grand Junction area (and associated counties) and occasional virtual visits are expected. Requires NP or PA board certification, active unrestricted licenses in coverage areas, and optional multi-state licenses. Primary workplaces are members’ homes; travel and in-home assessments are core to the role.
