Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical examinations, and diagnostic tests such as diabetic eye exams and spirometry. Utilize an iPad and clinical workflow to perform these assessments while connecting one-on-one with patients in their private residences. Provide comprehensive diagnostic and preventive services, including bone density screening, as ordered by the health plan, without prescribing medication or altering treatment regimens. Perform both in-person visits and virtual consultations as needed, traveling locally within licensed states to cover the Grand Junction MSA. Maintain flexibility to adapt to diverse home environments that may contain pets or allergens. Full-time role with 40 weekly hours, offering base pay ranging from $87,035 to $187,460 annually plus reimbursement for additional services.
