Job Summary
Provide and document skilled nursing care in accordance with physician orders and developed care plans for high-acuity home patients requiring feeding tube, med administration, tracheostomy, and ventilator support. Continuously assess patient conditions, report changes to supervisors or physicians, and implement nursing actions to prevent or reduce risks while adhering to infection control measures and OSHA standards. Educate patients and families on disease processes and self-care techniques, maintain accurate electronic documentation via patient tablets, and participate in ongoing educational programs to sustain clinical competencies.
Required Qualifications
- Active, unencumbered RN license in the state of practice
- Current CPR certification (with hands-on component)
- Current TB skin test (within last 12 months)
- Reliable transportation
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current, unrestricted state license as a Registered Nurse in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
- Ability to speak, write, read and understand English
- Ability to travel
- Ability to lift 50 pounds
- Visual and hearing acuity
- Strong sense of smell and touch
- Ability to sufficiently reposition patients and move equipment without assistance
- Ability to appropriately respond physically and mentally to emergency situations in the home or during transport
- Ability to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases (implied requirement for compliance)
- Required vaccinations and testing (subject to exemptions)
- One (1) year of pediatrics experience (if South Carolina)
- One (1) year of experience working under current nursing license (if California)
- One (1) year of experience working as a licensed nurse (if Louisiana)
- Continuing Education as required by state
- 2 years Registered Nurse experience with cardiac monitoring and oncology experience (if Medical Oncology RN)
- 2 years experience with vital assessment, plan of care development, and telephone triage (if Ambulatory RN)
- 2 years Registered Nurse experience (if Medical Surgical RN)
- 2 years of Registered Nurse, preferably with oncology experience (if Peri-Operative Care RN)
Desired Qualifications
- Six months prior hands-on nursing experience
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
